Home Services

The Home Services program provides homemaker services to those in need as a result of personal, family and health-related crises.

Home Services include home support, personal care, and caregiver relief.

Serving the Elderly and Homebound

The Home Services program provides service to consumers age 60 or older in Susquehanna County. The program is divided into specific cost centers-NFI, NFCE, WAIVER, NFCSP, & HSDF (See below). These services are provided by Personal Care Aides and include assistance with personal care and light housekeeping duties. Some consumers also receive skilled nursing visits on a quarterly basis. Referrals are received through the Susquehanna County Area Agency on Aging.

Human Services Development Fund

The Human Services Development Fund (HSDF) program is for consumers under the age of 60 who are considered homebound and require assistance with personal care or light housekeeping duties. Eleven HSDF consumers currently receive approximately 90 hours of personal care and homemaker services per month.

Nursing Facility Ineligible

Some 41 Nursing Facility Ineligible (NFI) program consumers receive personal care and homemaker services for approximately 400 hours per month.

Nursing Facility Clinically Eligible

The Nursing Facility Clinically Eligible (NFCE) program, for consumers considered nursing home level, has served three consumers with personal care and homemaker services for approximately 170 hours per month.

PA Department of Aging Waiver

The PA Dept. of Aging (PDA) Waiver program is steadily increasing, with 18 consumers receiving approximately 590 hours /month of personal care and homemaker services. This program is expected to increase by 75% over the next fiscal year. These consumers are considered nursing home level and require the greatest amount of care.

National Family Caregiver Support

The National Family Caregiver Support Program (NFCSP), a program for consumers of all levels of care, provided personal care and homemaker services to one consumer receiving approximately 18 hours/month.
